In-chat approval review: a composer takeover (priority 0) for file-changing approvals — true line diffs with word-level highlights, DISK-sourced context (host route, zero model reads), Split/Unified toggle, one-sided green creates, red deletion reviews — ONE FILE PER CARD (v0.17): the pending call's CONTIGUOUS same-file run merges into one diff with two file-scoped buttons, and cross-file or non-contiguous calls get their own card at their own turn, so nothing shown can ever be a phantom or stranded behind an unrelated decision. Doomed requests (missing/ambiguous operands, write-basis staleness) stay invisible and auto-forwarded. Everything else falls through to the native card.
